So my boss what’s us to fill ou a health history **** just in case something happens they can have it to give to emergency personnel. I have been taken antidepressants do I need to list them

    No you do not need to list them. They are looking for conditions such as diabetes ( in case you faint) epilepsy, heart problems. Serious stuff so they know how to treat you in an emergency. perfectly legal in the UK for your safety.

    JUST the basics . your name , your doctors name , if you are allergic to anything . if there is an emergency they can call your doctor or relative . this does not sound legal .

    ACA made it legal for your employer to accsess your medical info. While I have not heard of any cases where this is going on, lie and they run a check, then what?

    No you do not need to list them. Anything you put on that form is purely voluntary. You disclose things (or not) at your own discretion and risk.
