Yes. Any sign of blood in the urine is a cause for concern. Heavy amounts of blood, along with pain and nausea are a serious cause for alarm. You need to go to the hospital immediately.

Yes definitely. There's a possibility of internal bleeding, especially with those other symptoms. You should go to urgent care.

It may not be "serious" but it isn't normal and since it isn't going away on it's own i would see a doctor who may be able to prevent it from getting worse.
