Everywhere. Yams grow in Africa, and the Africans brought here as slaves saw sweet potatoes as the closest thing to yams, and called them yams. Most people in the US think they are the same thing.

Pretty much everywhere except the Botany and Agriculture Departments of Universities. I've never seen a real yam in the USA. And no, the difference is not the color of the flesh.

Everywhere, but I know the difference between the two and yams are nasty. I can eat sweet potatoes all day
