The next shot missed Billy's kneecaps by inches, going end-on-end, from the sound of it. to me it means: keeping going on without hitting anything thank you best answer shortly

A key phrase here is "by the sound of it" A bullet that is tumbling (going end over end) would make a very different sound from one that is rotating about its long axis as is nrmal

The writer probably meant "end over end", which means it is rotating from top to bottom around itself in a loop-de-loop
