Yes, the islands are built from the sea bottom up by under water volcanoes that erupt. The accumulation of lava eventually means there is some land above sea level. When the eruptions stopped, the island will be slowly eroded away by wind, rain and waves.

Yes, Hawaii is over a hot spot in the crust, and as the Pacific plate moves over that hotspot, a string of volcanic islands have formed.
