If someone says that x shop is on an island and not meaning an actual island, what do they mean?

It may mean one-of-a-kind or very rare.

You really should give us the context. It's not enough to say 'not meaning an actual island'; you have to give us some clue about the conversation.

A traffic island, perhaps. Surrounded by roads on all sides, maybe. An island in pavement rather than water.

Possibly it is the American way of saying that it is in the middle of a divided road. In Britain I have never heard of a shop being "on an island".
