You stop off at your computer to ask a bunch of strangers for advice. A taxi will get you to the hospital almost as fast and be a whole lot cheaper.

Get to an emergency room, you, your foot, and the spike (don't pull it out). If a bone gets infected, you'll lose the foot.

if you have time to write the question, don't call ambulance, but do go to urgent care/hospital. Do not take it out of your foot, it will cause more bleeding, or tearing on the way out.

Don't call an ambulance. Just go to the doctor. It isn't an actual emergency...is it? You can get to the doctor/hospital on your own without help...can't you? You did take the time to post this question...didn't you? Think about it.
