I need to know if the sentence below is grammatically correct and if it is not please rephrase for me thanks. Through the sacraments the priest can forgive my sins and give me Communion.

The grammar is correct; the content isn't. Priests act under the authority of God. And what Sacraments is the priest giving you when Communion is itself a Sacrament?

It's grammatically correct, but I don't know of any church that actually teaches that the priest can forgive sins. In the Roman Catholic tradition, at least, only God can forgive sins.
